I will try to answer your questions:
Do I have to select all accounts(P&L and BS)?
I think we didn't have a specific account selection, the fields for GL-account-selection we left empty.
Does it matter which account to use in "Bal. Sheet Acct: Opening"? Or do I have to create a dummy account?
It is up to you. If you have a suitable account, you can use it, if not - create one. We have created a new one.
Do I have to create a special document type or I can I use any?
I think it is not necessary to create a new one, I have just used a document type for GL-postings.
Does the report actually create documents in BKPF/BSEG? We are on classic ledger, not using New GL. If actual documents are created should these be reversed afterwards?

Should the periods need to be opened before running this report?
You don't have to open any specific periods.
Finally, the company code for which the extraction is required has a chart of accounts assigned to it and a country chart of accounts. Which chart of account should be selected in the report?
I would choose the country charts of accounts, but it could depend on your international company structures. The best way is to test it in the test system. If somethins goes wrong with this postings, you can reverse posted documents as described in the SAP-note: 2035887.
