Cool.. See MS kbase's on supporting MDAC with SQL 2012. Not fully compatible...
I suggest you download the MS SQL 2012 Client and update your reports to use the SQLNCLI11.dll. I know SQL 2008 client fully support SQL 2005 but not usre about 2012 client.
If you search on ReplaceConnection you'll find code sample on how to update them in your app. but I suggest you test that one reprot that fails by updating the Connection info using the Native 11 or 10 clients and see if that resolves the table name error.
In either case, MS doesn't supported that OLE DB Provider in SQL 2012 and CR works better with the SQLNCLI 10 client.
