if you are basis admin, you can check table E071 in SE16 for the following entries:
PGMID = "R3TR"
OBJECT = "TRAN"
OBJ_NAME = "ST12"
Is there any entry with OBJFUNC="D" there? This might me the transport request or package, where the tcode has been deleted or removed.
