hi Jennifer, here's one method that you may wish to try...and for this example we'll look at one field to start:

 

1) go to Insert > Crosstab and place it on your group header.

2) in the crosstab expert, insert Address as the Row value

3) insert the change date into your Summary but change the type of the Summary to Maximum

4) in the Customize Style tab, uncheck each of Column Totals on Top, Row Totals On Left and Show Cell Margins

5) press the Grid Lines and then uncheck Show Gridlines. press OK & OK again to exit.

6) supress the "Total" label at the top of the crosstab

7) suppress the Total label at the bottom of the crosstab as well as the summary of the date right beside the total label.

 

now you'll want to sort this crosstab by the change date:

 

1) right click on the crosstab and choose Group Sort Expert

2) change the sort to All and then choose your maximum date and choose Descending

 

now you'll have a record of all of the addresses for the person sorted by date.

 

next steps:   now you want to repeat the same process for Phone...Phone will have its own crosstab so Insert a Crosstab and use Phone for the Row field. i.e. each field will have its own crosstab in the group header. you can also suppress the Details section if you want now.
